Register For Sports Through FinalForms

We are very excited to announce that the Marysville School District Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

Parents and guardians will be able to review the data at any time to verify it is current and are required to sign the online forms once per year and after any update.

IMPORTANT: Parents and guardians should have received an account confirmation email from FinalForms. When you receive the email, please click the link and follow the steps to login and access your child(ren). You only need to do this if your child(ren) plans on participating in a sport. If you have not received an account confirmation email please contact liz_doyle@msd25.org or call her at (360)965-0111 because that means we don’t have an email listed in your child's Skyward account. An email is required before you can register.

Once you receive the email from FinalForms, click on the Parent Playbook to get started. If you require any support during the process, scroll to the bottom of the page and click “Use Support”.
